This form helps promote both your band and our show, so filling it out completely benefits everyone.
How to Fill It Out:
Adding Band Members & Crew: Click the small “+” sign (on the right or bottom) to add more members or website links. Every band member, including crew, needs to be listed for credentialing.
Band logo: required, upload a .png file.
Stage Plot: Even a rough sketch (like one on a bar napkin) is fine—just something to help us plan the setup.
Band Member Roles: Make sure to specify each member’s role so we know who to contact with staging or equipment questions.
Tax Information:

If your band is registered as a business, send a 1099 for the band.

If not, fill out and submit a 1099 for the person who will receive the payment.
